Patient Registration
Prior to your appointment, please complete the patient registration below and return it to our office with your referral letter. This will allow more time with your doctor.
For families who live more than 2 hours' drive from Sydney, a Sleep Clinic appointment with a sleep study on the same day can be organised. Your referring doctor can contact Dr Teng via fax on (02) 9382 0399 to make the necessary arrangements.
The Sleep Clinic appointment for new patients is usually for 45 minutes, and for existing patients it is approximately 20 minutes.
Fees
A Consultation Fee is payable on the day of the appointment by Cash, Credit Card or EFTPOS. For Medicare card holders Direct Credit to your account is available electronically on the day. Fees are negotiable for Pensioners with Bulk Billing available for the disadvantaged. Please discuss this with the Receptionist.
The clinic has a no-gap policy for Private Sleep Studies at state of the art facilities at St Luke's Hospital in Potts Point. This is an accredited paediatric sleep lab. A small reporting fee is charged instead at the time of the clinic appointment.
All sleep studies are personally reported by the physicians.
For Medicare eligible patients, there are no out of pocket costs if the sleep study is performed at the Sydney Children's Hospital.
Please note that a “no-show” fee might be charged if notification of cancellation is not received more than 24 hours before the scheduled appointment.
